Rewrite a bunch of general behavior

Rewrite a bunch of general behavior

All methods now throw NOT_SUPPORTED_ERR for unrecognized commands, and
INVALID_ACCESS_ERR if the particular type of access makes no sense for
the command (e.g., value for bold). copy/cut/paste can now throw
SECURITY_ERR. Lots of explanatory text rewritten.
