Re-ordered content and added a couple of markers for missing text as per ISSUE-59.
The Table of Contents has been regenerated (but the links may not work properly yet), but sections have not yet been renumbered, to assist following the changes.
There has been no text deleted, and only issue markers added, since the last version.
In subsequent commits the sections will be renumbered, and content edited if required to simplify reading.
